If this is your first time at the clinic, please arrive 15 minutes before your appointment to fill out initial paperwork. If this is a return visit, please arrive 5 minutes before your appointment to ensure your appointment starts on time incase any follow up paperwork or assessments need to be performed. 24 hours notice is required for ALL cancellations or appointment rescheduling. A no show fee will apply to all appointments where the client does not show up for the appointment in the amount of their appointment. A no show is considered if you are 20 mins or more late for your appointment. Last minute cancelations (less than 24 hours notice) require 50% of the service to be paid before your next appointment can be booked. No Shows will not be rebooked until payment for the missed appointment is paid in full. Debit, credit, Apple Pay, Google Pay, and cash payments are accepted. Online billing is available for most insurance companies.
